The effect of two nights of sleep restriction on emotional attention – a MEG study

Background
Sleep loss is associated with changes in emotional functioning which extend beyond self-reported mood. However, results from neuroimaging studies are puzzling, with some showing increased responses towards negative pictures, while others show increased responses to neutral pictures, in both cases suggested to be driven by deficits in executive control. The aim of this magnetoencephalography (MEG) study was to put previous findings to the test and investigate if two nights of sleep restriction altered attention allocation towards negative or neutral pictures, and if this was influenced by executive control of attention.
Thirty-three healthy, well-sleeping participants were included in this cross-over within subject’s designed study and were tested once after normal sleep and once after two nights of sleep between 01:00 and 05:00. In the experimental task the participants were presented with an arrow indicating which direction to covertly focus attention (executive control), followed by two simultaneously presented pictures (emotional attention), one Unpleasant and one Neutral, tagged with frequencies (left: 30Hz, right: 40Hz). After the pictures, a dot appeared to the left or right and the participant was to respond on which side the dot appeared. During the task we measured the neurophysiological response using MEG. The main outcome was the visual Steady State Responses (vSSR) from the 30Hz and 40Hz frequencies during the picture presentation. We also assessed subjective sleepiness using the KSS, sustained attention using the PVT, and positive and negative affective state using the PANAS.
Results
Results showed that sleep restriction increased sleepiness, impaired sustained attention and lowered positive mood state. We also confirmed that covert attention was successfully directed in the arrow direction, especially in visual cortex as indicated by an increase of up to 40% in vSSR power. However, sleep restriction did not alter the attention towards Unpleasant or Neutral pictures (emotional attention). Sleep restriction did also not alter attention allocation towards the arrow direction (executive control). Furthermore, attention allocation did not affect response times to the subsequent dot.
Taken together, using a sleep restriction protocol we could not resolve the puzzling responses to affective stimuli found after total sleep deprivation, as we found neither an increased response towards negative nor neutral pictures. It is possible that the two nights of sleep restriction have a less strong effect on emotional functioning than one night of total sleep deprivation. The study contributes with novel methodology, assessing the effects of sleep restriction on emotional attention using MEG and vSSR.
The effects of a sleep robot intervention on sleep, depression and anxiety in adults with insomnia – A randomized waitlist-controlled trial

Introduction
Insomnia is one of the most common sleep disorders in adults. The hyperarousal model of insomnia emphasizes heightened arousal as an important causal and maintaining factor in insomnia. Different relaxation techniques have been found to reduce hyperarousal and improve sleep. The Somnox sleep robot is promoted as sleep enhancing through relaxation by breath guidance. The aim of the current study was to assess if a three-week intervention with daily at-home use of the sleep robot had effects on symptoms of insomnia and/or concurrent symptoms of depression and anxiety in adults, compared with a waitlist-control group. There are currently no independent scientific studies on the sleep robot, hence the study fills a gap regarding the robot’s efficacy.
Materials and methods
A total of 44 participants (mean age: 48.91, female: 79.55%) who met the DSM-5 diagnostic criteria of chronic insomnia were recruited to the study. The participants were randomized to a three-week intervention with the Somnox sleep robot, or to a waitlist control group. The primary outcome measure was the Insomnia Severity Index administered at baseline, mid-intervention, and post-intervention. Secondary outcome measures were the Pre-Sleep Arousal Scale and the Hospital Anxiety and Depression Scale. Additionally, sleep onset latency, wake time after sleep onset, total sleep time, and sleep efficiency, were measured both subjectively with the consensus sleep diary, and objectively with actigraphy. Mixed-effects models were used to analyze the data. Further, adherence to the intervention was measured. Spontaneous comments about how the participants experienced the robot were logged. The prospectively published study protocol and statistical analysis plan was followed.
Results
The effect of the sleep robot intervention on the participants’ level of insomnia symptoms was not statistically significant. The differences between the intervention group and the control group on the measures of arousal, anxiety and depression were also not statistically significant. Neither were the sleep diary and actigraphy variables, except for total sleep time measured with actigraphy, in favor of the control group. The participants enclosed both positive and negative experiences with the sleep robot.
Conclusions
A three-week intervention with daily at-home use of the Somnox sleep robot was not found to be an effective method to relieve symptoms of insomnia in adults. Possible roads for further exploration of the sleep robot are studies on sleep disorders other than insomnia, participants of older or younger ages, and studies conducted under more controlled conditions (i.e., a sleep laboratory).
Night-to-night variability of automatically derived physiological endotypic traits in patients with Obstructive Sleep Apnea

Introduction
Emerging data suggested physiological endotypic traits (e.g., loop gain) contribute to precision medicine in obstructive sleep apnea (OSA). However, the clinical utilities of endotypic classification have been limited by technical complexity. In addition, the night-to-night variability of derived endotypic traits has not been extensively studied. In the current project, we implemented an established endotyping methodology into clinical polysomnographic studies and studied the night-to-night variability of endotypic traits in moderate-to-severe OSA patients.
Materials and Methods
A previously published methodology ([1]) for derivation of physiological endotypic traits in OSA from standard polysomnographic sleep recordings was adapted and refined for use in polysomnography data sets recorded with the EMBLA A10 system. Detailed information from nasal flow, apnea/hypopnea detection, and arousal classification was used in the analysis. The new algorithm was validated in comparison with the previously published implementation on a set of simulated data based on the underlying model of the ventilatory control system. The evaluation of night-to-night variability was performed using data from a clinical trial with 2 consecutive in-lab overnight polysomnographic recordings. Data has been evaluated by comparing the performance in terms of recognizing the endotypic trait loop gain (LG) as a proof-of-concept.
Results
Using simulated data, both algorithms performed almost identically (p<0.001, ICC=0.99), confirming the validity of the underlying modeling technique. Night-to-night variability in LG was moderate for the new implementation (p < 0.001, ICC = 0.71), with a mean difference of 0.005 (95% CI: -0.04 – 0.05).
Conclusions
Automatic derivation of physiological endotypic traits from polysomnographic recordings is feasible and reproducible. LG varied moderately between consecutive nights, indicating that a single night assessment of endotypes may be sufficient to support clinical decision-making. Using the new implementation of the previously described technique, we were able to adjust the processing according to the devices used in our protocols, eliminate various technical limitations, and reduce overall variability.

Phenotypic differences in psychiatric symptoms across the day are predicted by someone’s psychiatric profile and chronotype

Background
The strong circadian influence in psychiatric disorders is leading the way to unleash new opportunities to increase efficacy of personalised psychiatric treatment. In order for such opportunities to be successful in psychiatry, there is a need to better characterise when individuals function and feel worse across the day.
In the current pre-registered study, participants (N = 503) completed self-reported measures of psychiatric traits (13 scales) and chronotype (rMEQ) at baseline. Of those, 441 continued the next day with repeated ratings of psychiatric symptoms across the day (~08:00-01:00). Generalised additive mixed-effects models were used to assess the extent and shape of any non-linear interaction between transdiagnostic psychiatric constructs, time of day, and chronotype.
Results
Results show that the 13 psychiatric traits loaded onto three factors: depression-anxiety; downregulatory problems; social dysfunction. The 22 symptoms and activities rated across the day loaded onto four factors: emotional symptoms; fatigue; ADHD-type symptoms; activity. These four factors showed the expected diurnal pattern. For fatigue, this pattern shifted as a function of chronotype: evening types showed higher levels of fatigue in the morning (~8:00-10:00) as compared to morning types and this relationship reversed in the evening (~20:00 and later). Morning and evening types further markedly differed in when ADHD-type symptoms were most prevalent and this differed for high versus low psychiatric traits. Morning types high in depression-anxiety or downregulatory problems showed worse ADHD-type symptoms compared to their evening type equivalents throughout the whole day. However, in those low in depression-anxiety or downregulatory problems, evening types were more vulnerable for ADHD-type symptoms in the morning. This pattern had reversed by the evening (~20:00) such that morning types were more vulnerable for ADHD-type symptoms. Regarding activity, in the late evening (~24:00) evening types low in downregulatory problems were more active than morning types, while morning types were overall more active throughout the day but showed a sharper decrease in activity in the evening.
These findings show that psychiatric symptoms vary across the day depending on an individual’s psychiatric profile and chronotype. In all, chronotype should be considered in personalised medicine and when tailoring treatments and structuring workload to meet the optimal time of day for the individual.
Effect of sulthiame on sleep quality assessed by cardiopulmonary coupling in patients with OSA – a RCT

Objectives
We studied the effect of STM on sleep quality assessed by cardiopulmonary coupling (CPC) in moderate-to-severe OSA patients.
Methods
Post-hoc analysis of a double-blind, placebo-controlled, phase IIb trial (n=58, 72% male, age 60±10 years, body mass index 28±3 kg/m², AHI 56±22 events/h). Patients were randomized to receive placebo (n=22), STM 200 mg (n=11), or STM 400 mg (n=25), respectively. Polysomnography recordings were performed at baseline and after 4 weeks of intervention. The electrocardiography signal from sleep studies was used for CPC analysis. The proportion of high-frequency coupling (HFC, stable sleep), low-frequency coupling (LFC, unstable sleep), elevated-LFC narrow-band (e-LFC_NB, marker of periodic breathing) and elevated-LFC broad-band (e-LFC_BB, sleep fragmentation) was calculated. Quality of life was assessed by Functional Outcomes of Sleep Questionnaire (FOSQ). The changes in CPC variables between STM 200/400 mg and placebo from baseline to post-treatment were analyzed using analysis of covariance.
Results
Baseline AHI and CPC parameters did not differ between groups. Compared to placebo, STM 200 mg significantly reduced LFC (0.9±11.4 vs.12.2±16.0%, p=0.026) and e-LFC_BB (0.4±8.5 vs. 9.1±4.5%, p=0.003). The corresponding changes for STM 400 mg were 6.7±11.3% and 4.8±10.5%, (p=0.089 and 0.13, respectively). HFC and e-LFC_NB did not change significantly by the STM treatment. Improving HFC, LFC and e-LFC_BB were associated with improvement of FOSQ total score after STM treatment (n=36, Spearman correlation, r=0.44, 0.40 and 0.36, p=0.007, 0.015 and 0.032, respectively).
Conclusions
4-week treatment of STM 200 mg was associated with improved sleep quality assessed by CPC. Nocturnal CPC analysis may provide unique information on patient-reported outcome measures in OSA patients. (EudraCT: 2017-004767-13)

Health risks related to polyurethan foam degradation in CPAP devices

Background
A recent medical device recall notification from Philips Respironics reported on risk for degradation of polyurethane foam (PUF) in CPAP devices used for obstructive sleep apnoea (OSA) treatment.
Methods
A national, longitudinal cohort study on OSA patients in Sweden using CPAP over an 8-year observation period with health data collected from Swedish national registries. The brands of CPAP devices included in the recent medical device recall notification were defined as PUF-CPAP (N=18,561) and other brands as non-PUF-CPAP (N=29,830). Main outcomes were anti-obstructive drug use, hospitalizations, cancer incidence, and mortality. Statistical methods included propensity score matching and multivariable regression models clustered by county and adjusted for anthropometric data and comorbidities.
Results
During 139,056 person-years of follow-up, PUF-CPAP use was associated with more frequent use of short-acting beta2-agonists (SABA) and oral corticosteroids (OCS). During the first year after CPAP initiation, 3.3% vs. 2.7% (p=0.001) had upwards of 3 collections of SABA and 10.3% vs.9.1% (less than 0.001) were prescribed OCS, corresponding to an adjusted odds ratio 1.24 (95% Confidence Interval 1.10 to 1.39) and 1.10, (1.00 to 1.23) respectively. Mortality and overall cancer incidence were similar. Incident lung cancer risk was increased in the PUF-CPAP group (0.3% versus 0.1%, less than 0.001), but this finding was not robust in sensitivity analysis.
Conclusions
Use of PUF-CPAP was associated with mild deterioration of obstructive lung disease control, while overall cancer and mortality risks were unchanged. Signals of increased lung cancer incidence were inconclusive. Our findings may guide health care professionals and patients on risk-benefit assessment of PUF-CPAP use and replacement policies.
Kan ökad ljusexponering för äldre förbättra deras sömn, hälsa och funktionsförmåga? – En interventionsstudie

Med en allt längre medellivslängd i samhället är de viktigt att förebygga ohälsa och främja äldres livskvalitet. Forskning visar att äldre vistas sällan i dagsljus och har mörk vistelsemiljö. Ljuset reglerar sömn och vakenhet, humör, prestation och långsiktig hälsa. Utveckling av belysningslösningar med hälsa och välbefinnande i centrum har gått snabbt men ännu finns få lösningar utvärderade i Norden. I studien jämförs två olika belysningslösningar i äldreboende. I studien jämfördes en ny dynamisk ljusdesig inklusive en stark ljuspuls (6500 Kelvin, dagljuslik på förmiddagen) mot en traditionell programmering med ljusnivå (3000 Kelvin, gult ljus). I en cross-over design studerades äldre under båda ljusbetingelserna efter 6-8 veckor. Effekter på grupper med antingen demensdiagnos eller somatisk diagnos studeras. I de två studierna fick deltagare bära ljus- och aktivitetssensor och utfallsvariablerna var sömn, sömnkvalitet, dagtidsaktivitet, dygnsrytmanalys. Skattningar av trivsel, synergonomiska aspekter, livskvalitet och mental hälsa gjordes via strukturerad intervju. Trettiotvå boende studerades (varav 12 män) där medelåldern var 84,9 år (68-97 år).
Slutsats
Resultatet för index för depressivitet, gav vistelse i det starkare ljuset mindre symtom. Men i övriga index som berörde sömnproblem och dagtidssömnighet upplevdes ingen skillnad.
Några signifikanta skillnader mellan svagt och starkt ljus erhölls inte i ljuskvalitet. Det är också påtagligt hur lite ljuset bidrog till symtom som huvudvärk, irritation som kan uppstå av brister i ljuskvalitet. De boende anpassade sitt allmänljus till sin dag och upplevde inte att det blev för ljust i rummet och möjlighet till läsning var god. I mindre grad bidrog ljuset till känsla av pigghet, tillskott av energi och som bidrag till avslappning. Man håller i stort med om att ljuset bidrar till att skapa en plats de gillade att vistas på och att man i stort sett kunde rekommendera både den starkare och svagare ljuslösningen till andra boenden.
Boende bar aktivitetsklocka under två mätveckor. Det starkare ljuset bidrog till en senare sömnstart om 18 minuter och sovtiden minskade med 20 minuter. På eftermiddagen var de som exponerades för ny ljusprofil mer aktiv i sitt rörelsemönster. Ljusexponeringen varierade under mätveckor men inga statistiskt säkerställda skillnader erhölls mellan det svaga och det starka ljuset. Ljusintaget blev lägre för boende med lägre grad av rörlighet uppmätt med rörelsemätning. Tid för sänggående och i högre grad tiden för uppstigande blev förskjuten (senarelagd).
Studien ger ny kunskap och råd för hälsosam belysning för äldre och kan bidra till bättre livskvalitet för personer vid äldreboende.
Vilket stöd önskar skolungdomar för sin sömn?

Bakgrund
Sömnsvårigheter ses hos en stor andel ungdomar både nationellt och internationellt och bristande sömn betraktas som ett folkhälsoproblem (Gradisar, Gardner & Dohnt 2011; Norell-Clarke & Hagquist 2017; Saxvig et al., 2020). För lite sömn kan påverka hälsa, välbefinnande och utbildning i negativt riktning (Hirshkowitz et al., 2015). I det främjande arbetet med ungdomars sömn är det väsentligt att fråga ungdomarna själva vilket stöd de skulle önska. Denna studie utforskar därför ungdomars förslag på hur deras sömn kan stödjas.
Metod
Data samlades in via åtta fokusgruppsintervjuer med 43 ungdomar i årskurs nio. Data analyserades med kvalitativ innehållsanalys (Lindgren, Lundman & Graneheim, 2020).
Resultat
Resultaten beskriver ungdomarnas förslag om hur deras sömn kan stödjas i tre teman:
-Få stöd av involverade föräldrar
Trots att ungdomarna börjar bli självständiga individer vill de ha stöd för att sova bättre. Framför allt önskar de stöd från sina föräldrar i form av rutiner och trygghet.
-Få stöd för att uppnå kunskap om sömnens “varför” och “hur”.
Ungdomar önskar mer och djupare kunskap om sömnen för att kunna göra välgrundade val i relation till sin sömn. De värdesätter att få kunskap och råd från någon som de har en relation till och litar på, då upplevs råden som riktade och personliga. Ungdomar vill att sömnundervisningen i skolan ska vara mer interaktiv, de lär sig genom att diskutera och reflektera tillsammans.
-Bli guidad till att hitta balans
Ungdomar upplever att fysisk aktivitet gynnar deras sömn, därför önskar de att uppmuntras till aktivitet hemma och i skolan. Den tystnad som infinner vid sänggåendet är svår att hantera – tankar, oro och rädsla tar över. Ungdomar önskar bli guidade till att hantera tystnaden. Förutom balans mellan aktivitet och vila och mellan ständig input och tystnad så önskar ungdomar stöd för att hitta balans mellan skolarbete och fritid. De upplever att balans under dagen gynnar deras sömn.
Slutsats
Det stöd ungdomar önskar är; involverade föräldrar, kunskap om sömnens ”varför” och ”hur” samt vägledning för att hitta balans. Enligt ungdomarna spelar föräldrarnas rutiner, trygghet och vägledning en nyckelroll i förhållande till deras sömn, därför bör föräldrar involveras i det sömnfrämjande arbetet, samtidigt som ungdomarnas ökade självständighet tas i beaktande. Genomgående i studien beskriver ungdomarna en önskan att få kunskap och stöd som riktar sig till dem som individer. För detta ändamål måste ungdomar bli genuint lyssnade på; då ges möjlighet att påverka och förebygga den enskilde ungdomens sömnsvårigheter.
Treating and preventing depression by addressing insomnia – long-term follow-up of three RCTs

Comorbid insomnia and depression is common, but seldom adequately treated. Cognitive behavioral therapy, CBT, for depression is recommended for these patients in most guidelines, but the most common treatment is antidepressants, sometimes combined with sleep medication. Studies show that this is not adequate to treat insomnia and thus prevent relapse into depression. We present data from three RCT’s on insomnia with depressive symptoms:
1. Participants (43) with insomnia and major depression received either therapist-guided Internet-based cognitive behavioral therapy for insomnia (ICBT-i) or therapist-guided Internet-based cognitive behavioral therapy for depression (ICBT-d). Results show that the two treatments were equally effective in reducing depression, but ICBT-i was significantly more effective in reducing insomnia. The results were maintained at the 36 months follow-up [1].
2. Participants (143) with insomnia but without major depression received either ICBT-i or an active Internet-based control treatment (ICBT-ctrl). Post treatment and at the 6 months follow-up, ICBT-i was more effective in reducing insomnia severity. For the whole sample, persistent good sleep at post-, 6- and 12 months follow-up predicted significantly less depressive symptoms at the 36 months follow-up [2].
3. Participants (126) with insomnia and major depression received either a new combination treatment, encompassing methods from both CBT-i and CBT for depression, or depression treatment with a placebo intervention for insomnia. Preliminary results imply the new treatment is superior to the control treatment.
Conclusions
These results underline the importance of providing evidence based insomnia treatment (CBT-i) to patients with co-occurring insomnia and depression.
Sömnrestriktion vs sömnkomprimering: Preliminära resultat från en randomiserad kontrollerad studie

Bakgrund
Sömnrestriktion är en central komponent av kognitiv beteendeterapi för insomni, och har även visat goda resultat när det använts som en självständig behandling för insomni. Sömnrestriktion är en effektiv intervention, men också associerad med oönskade sidoeffekter såsom huvudvärk och extrem trötthet. Syftet med denna studie var att jämföra sömnrestriktion med en potentiellt mindre krävande intervention – sömnkompring. Detta är den första randomiserade, kontrollerande studien i världen som jämför dessa två interventioner som behandlingar för insomni.
Deltagarna (n=234) var vuxna med insomni bosatta i Sverige. De randomiserades till 5 veckor behandlarstödd internetförmedlad kognitiv beteendeterapi bestående av antingen sömnrestriktion (n=117) eller sömnkomprimering (n=117), följt av 5 veckors fortsatt internetförmedlad egenvård. Uppföljning gjordes 10 veckor efter behandlingsavslut. Primärt utfallsmått var svårighetsgrad av insomni, mätt med Insomnia Severity Index före behandlingen, veckovis under de första 5 veckorna, vid post samt vid uppföljning efter 10 veckor och ett år. Sekundärt utfallsmått var sidoeffekter av behandlingarna som mättes med skalan Adverse Events, som är framtagen för att mäta negativa effekter av insomnibehandling.
Preliminära analyser visar att båda grupperna förbättrades med avseende på svårighetsgrad av insomni från pre till post, och att det vid post inte var någon signifikant skillnad mellan de båda grupperna. Negativa sidoeffekter förekom i båda behandlingarna vid behandlingsvecka 2 och 4, men dessa var något vanligare i sömnrestriktionsgruppen där fler deltagare rapporterade biverkningar.
Slutsats
Slutsatserna är att dessa preliminära data indikerar att sömnrestriktion och sömnkomprimering ger jämförbara resultat vid behandling för insomni, samt att sömnrestriktion initialt är förknippat med fler negativa sidoeffekter. Dock återstår att analysera data från uppföljningsmätningarna. Om resultaten står sig, innebär det att sömnkomprimering är ett fullgott alternativ till sömnrestriktion och kan erbjudas patienter som inte kan eller vill genomföra sömnrestriktion
Jakten på regnbågen: En systematisk litteraturöversikt om demografiska data för sexualitet och könsidentitet i studier med kognitiv beteendeterapi för insomni

Introduction
Kognitiv beteendeterapi för insomni (KBT-I) är ”golden standard” vid insomni men det finns obesvarade frågor gällande för vem behandlingen har önskad effekt. Svenska rapporter har visat att personer som tillhör minoriteter ifråga om sexuell läggning eller könsidentitet sover sämre än personer som tillhör majoriteten. Sömnproblem är förknippat med olika typer av ohälsa, inklusive sämre förmåga att hantera stressorer. Personer inom HBTQ+ har en extra belastning i form av minoritetsstress (att hantera andras okunskap, negativa attityd och en ökad risk för verbalt eller fysiskt våld). Effektiva insatser vid insomni är därför särskilt viktiga för personer inom HBTQ+. Syftet med studien var att genomföra en metaanalys på randomiserade kontrollerade studier med KBT-I för att besvara frågan om vilken effekt KBT-I har för personer inom HBTQ+.
Metod
Sökningar i databaserna PubMed och PsycInfo genomfördes 2021 för att hitta RCT-studier om KBT-I som publicerats 2020 och bakåt. Sökningarna gav 691 unika träffar som granskades för att avgöra om de uppfyllde inklusionskriterierna. Inkluderade artiklar lästes och kodades för att identifiera om de rapporterade demografisk data som kan relateras till HBTQ+.
Resultat
Av de 241 artiklar som uppfyllde inklusionskriterierna, rapporterade endast 2 studier demografisk data om deltagare inom HBTQ+. Underlaget var för litet för att en metaanalys skulle kunna genomföras. I de två studierna som redovisade tillhörighet till HBTQ+ redovisades inte behandlingens resultatet separat för dessa deltagare. Det beskrevs inte heller hur deltagarna tillfrågats om sexualitet och könsidentitet, eller för vilka ändamål data samlats in. I de studier som inte rapporterat tillhörighet till HBTQ+ beskrevs inte om deltagarna tillfrågats om detta.
Slutsats
Det är okänt vilken effekt KBT-I har på sömnen för personer inom HBTQ+. Detta är bekymmersamt då personer inom HBTQ+ i lägre grad söker vård och oftare upplever missnöje med vården samtidigt som de har en högre risk för stressorer och sömnproblem. Inom smärtbehandlings-forskningen skapas riktlinjer för att studier ska rapportera fler demografiska data, för att behandlare bättre ska kunna avgöra för vem behandlingen kan tänkas passa. Kanske är det läge för sömnforskningen att följa efter?
Genomförbarhet och preliminära effekter av KBT för insomni på en psykiatrisk öppenvårdsmottagning

Bakgrund
Mellan 50-80 procent av patienterna i psykiatrin har sömnproblem av insomnityp. Förutom minskad livskvalitet och försämrad funktionsförmåga kan sömnproblemen förvärra annan psykiatrisk problematik och öka risken för återfall i till exempel depression. Enligt internationella riktlinjer ska kognitiv beteendeterapi för insomni (KBT-i) användas som förstahandsval för behandling av sömnproblem. I praktiken är det dock ovanligt att psykiatrins patienter erbjuds KBT-i, i stället behandlas de flesta med läkemedel. Det saknas också forskningsstudier där KBT-i utvärderas i reguljär klinisk verksamhet.
I den här pilotstudien undersökte vi genomförbarheten av en gruppbehandling med KBT-i på en psykiatrisk öppenvårdsmottagning i Stockholm för patienter med depression, bipolär sjukdom II, ångestsyndrom och PTSD. Vi undersökte också om symtom på insomni, depression och ångest förändrades efter behandlingen.
Metod
18 patienter med självupplevda sömnproblem inkluderades till en gruppbehandling med KBT-i. Behandlingen bestod av sex sessioner och löpte över sex veckor. Primärt utfallsmått var klinisk genomförbarhet. Detta definierades som att patientintresset skulle vara tillräckligt stort för att kunna genomföra minst en grupp per termin (ungefär 8 patienter), minst hälften av de inkluderade patienterna skulle delta vid första sessionen, deltagarna skulle delta vid minst hälften av sessionerna och mindre än hälften av deltagarna skulle hoppa av från behandlingen. Vidare skulle gruppledarna uppleva behandlingsmanualen som trovärdig, effektiv och möjlig att fortsätta använda efter studiens avslutande. Sekundära utfallsmått var förändringar i insomnisymtom, samt symtom på depression och ångest. Symtomen mättes före och efter behandlingen med Insomnia Severity Index (ISI), Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder Scale (GAD-7).
Resultat
Behandlingen var genomförbar i reguljär klinisk verksamhet. I genomsnitt 11 patienter per termin ville delta i behandlingen, 89 procent av patienterna kom till första sessionen och de deltog i snitt vid 5 av 6 sessioner. 5,5 procent hoppade av behandlingen. Gruppledarna uppfattade behandlingsmanualen som trovärdig, effektiv och ville fortsätta använda den även efter att studien var avslutad. Symtom på insomni och depression minskade efter behandlingen och det fanns en trend mot en minskning av ångestsymtom.
Slutsats
Gruppbehandling med KBT-i är en lovande behandlingsform för att behandla sömnproblem inom psykiatrin. Behandlingen är genomförbar i den kliniska verksamheten och resultaten pekar på att den har en positiv effekt på symtom på insomni och depression. Resultaten behöver bekräftas i en randomiserad, kontrollerad studie.
Does the facial expression matter for the attention of the sleep deprived? An experimental study in young adults

Background
Despite reduced attentional resources, acute sleep loss promotes the brain’s response to increasingly negative stimuli. Whether angry and fearful faces, possibly indicating a social threat, can prevent the drop in attention due to sleep loss is not well-researched. It is also unknown whether sleep-deprived individuals evaluate the social appeal of faces differently. Therefore, we conducted a within-subjects study, where 43 young adults participated in one night of total sleep deprivation and one night with an 8-hour sleep opportunity in counterbalanced order. On the morning following each condition night, an eye tracker was used to present participants with images of happy, angry, fearful, and neutral faces. Participants also evaluated each face’s attractiveness, trustworthiness, and healthiness. Following sleep loss, the total dwell time on faces was shorter than after sleep, irrespective of the displayed emotion (happy: -11.1%; neutral: -8%; fearful: -9.5%; and angry: -13.8%; all P(less than)0.001 for the sleep main effect; P=0.222 for sleep*emotion interaction).
Results
When examining the relative distribution of time spent on the different regions of each face, we found that the relative time on the eye region of happy, neutral, and fearful faces was reduced following sleep loss (p(less than)0.05). In contrast, the relative time on the eye region of angry faces did not differ between experimental conditions. Finally, faces were overall evaluated as less trustworthy (-2.4%) and attractive (-3.2%) after sleep loss (p(less than)0.05). Altogether, our eye-tracking study does not provide compelling evidence for the hypothesis that sleep loss is associated with increased visual attention for socially threatening stimuli, such as angry and fearful faces. The finding that the visual exploration pattern of angry faces remained unaffected by sleep loss may indicate that the brain uses similar strategies as after sleep to visually process potential social threats. Finally, the sleep loss-induced drop in social appeal of others appears ecologically insignificant.
Experimental effects of sleep loss and low-grade inflammation on habitual and goal directed learning

Background
Behavioural control arises from a balance between model-based and model-free control. Model-based control is cognitively costly but enables adaptation to environmental changes and therefore allows higher goal-directed control. In contrast, model-free control is fast, cognitively inexpensive, but inflexible and relies more on habitual learning. Overreliance on model-free control and reduced model-based control is found across various mental health conditions, suggesting that these modes of control may be influenced by common transdiagnostic processes. Since insufficient sleep and low-grade inflammation are commonly seen across mental illnesses, this study investigated how they by themselves and in combination influence behavioural control.
The abstract is based on the first 36 individuals that have completed an ongoing experiment. The study included three sessions: a sleep loss (2 nights of 4 hours of sleep) and a normal sleep condition (within-subjects) and a low-grade inflammation condition (COVID-19 vaccination), preceded by either sleep loss or normal sleep (between subjects). Sickness symptoms were assessed using the SicknessQ and a sequential decision task was used to quantify model-based and model-free control.
Results
Results show highest sickness symptoms after vaccination in combination with sleep loss (M = 35.2, SD = 17.1), followed by sleep loss (M = 22.7, SD = 12.7), vaccination with normal sleep (M = 21.9, SD = 15.3), and normal sleep (M = 13.4, SD = 8.0). Model-free control was higher in the vaccine condition as compared to the normal sleep condition (Mdiff = 0.12, 95% CI: 0.01-0.23, p = .017), but this was not significantly modulated by sleep condition. Model-based control did not significantly differ between the conditions.
The tentative results (final analyses will include more subjects) show that low-grade inflammation may increase percentage of model-free control, making people rely more on habitual learning. This data indicates a role for inflammation as a mechanism underlying altered learning methods in goal-directed processes seen in psychiatric disorders.

Ett svenskt instrument för att bedöma barns sömn: Pediatric Insomnia Severity Index. Översättning, validitet och reliabilitet

Bakgrund
Sömnstörningar har blivit ett vanligt folkhälsoproblem och sträcker sig allt längre ner i åldrarna. Pediatric Insomnia Severity Index (PISI) innehåller sex frågor baserade på ICSD-II kriterier för insomnia och beskriver föräldrars upplevelse av barns sömn under den senaste veckan.
Syftet med studien var att presentera en validerad och reliabilitetstestad svensk version av PISI.
Material och metod
Den engelska versionen av PISI översattes till svenska, diskuterades och översattes därefter återigen tillbaka till engelska. Föräldrar till friska barn i åldern 3-10 år besvarade den svenska versionen av PISI och KIDSCREEN- 27 vid två tillfällen. Explorativ och bekräftande faktoranalys utfördes av svaren vid första tillfället, och mellan svaren vid första och andra tillfället. Strukturerad ekvationsmodellering, och korrelationer mellan PISI och KIDSCREEN-27 utfördes och analyserades.
Resultat
Frågeformulären fylldes i av 160 föräldrar vid första tillfället, varav 100 föräldrar fyllde i vid andra tillfället. Barnens medelålder var 6,9 år (sd ± 2,2, åldersspann 3,0-10,7 år), 44 procent var flickor. Den bekräftande faktoranalysen hittade två korrelerade faktorer, insomningssvårigheter (SOP) och svårigheter att behålla sömn (SMP). Den svenska versionen av PISI visade sig vara bra konstruerad och ha god reliabilitet. De två faktorerna av PISI påverkade alla dimensionerna av KIDSCREEN-27.
Slutsats
Den svenska versionen av PISI kan användas för screening av sömnproblem hos barn i åldern 3-10 år. PISI är ett bra hjälpmedel att använda vid samtal med föräldrar om barns sömn och vid utvärdering av behandling. PISI är även en tillgång för forskare som studerar barns sömn såväl inom hälsovård som psykiatrisk och somatisk sjukvård.
Kort sömnlängd, problem med att vakna och upplevd dålig familjeekonomi under barndomen är kopplade till symtom på insomni hos ungdomar – en longitudinell studie

ABSTRACT
Otillräcklig sömn är ett folkhälsoproblem som påverkar den psykiska och fysiska hälsan hos barn och ungdomar. Klagomål om sömnbesvär är särskilt vanligt bland ungdomar. Denna longitudinella studie syftade till att undersöka huruvida sömnlängd, väckningssvårigheter, trötthet i skolan och familjens ekonomiska situation bland flickor och pojkar på låg- och mellanstadiet (6-10 år) kan förutsäga symptom på insomni hos tonåringar på högstadiet och gymnasiet (14-16 år). Barn på lågstadiet och mellanstadiet (n = 522, 49,8 % flickor) i södra Sverige besvarade ett frågeformulär vid baslinjemätningen (9,4 år, SD 1,28 år) och vid uppföljning på högstadiet och gymnasiet (ålder 14,3, SD 0,69). Logistiska regressionsanalyser användes för att undersöka om trötthet i skolan, problem med att vakna, kort sömnlängd och upplevd familjeekonomi vid baslinjen förutspådde symptom på insomni vid uppföljning. Resultatet visade att upplevd ganska dålig/mycket dålig familjeekonomisk situation (OR 3,3) och kort sömnlängd (less than 10 timmar) (OR 2,3) bland flickor vid baslinjen var associerade med symtom på insomni vid uppföljningen. Att ha problem med att vakna bland pojkar vid baslinjen var associerat med symtom på insomni vid uppföljning (OR: 3,6). Kort sömnlängd, problem med att vakna och upplevd dålig familjeekonomi under barndomen var kopplade till symtom på insomni hos ungdomar. Skillnader mellan flickor och pojkar i föreliggande studier motiverar ytterligare studier för att effektivt kunna ge stöd till ungdomar med sömnbesvär.
A brief screening instrument for insomnia in adolescents –
measurement properties of the Minimal Insomnia Symptom Scale (MISS)

Background
The Minimal Insomnia Symptom Scale (MISS) is a three-item screening instrument that has been found to be psychometrically sound and capable of screening for insomnia among adults and older people. This study aimed to test the measurement properties of the MISS together with an additional item focusing on daytime functioning among adolescents using the Rasch measurement model.
Methods
A cross-sectional design was used, and data from adolescents (age 13-17 years, n=3022) were analyzed using the Rasch measurement model.
Results
The MISS had good measurement properties. When adding the item “daytime disturbance”, the measurement properties deteriorated. When replacing the original MISS item “not rested by sleep” with the item “daytime disturbance”, the measurement properties slightly improved. We label this new scale the MISS-Revised (MISS-R). The reliability was better for the MISS-R (0.55) compared to the MISS (0.50). The optimal cut-off was found to be >6 points, both for the MISS and the MISS-R.
Conclusions
This study provides general support that both the MISS as well as the MISS-R have good fit to the Rasch model. At this stage, neither the MISS nor the MISS-R can be advocated over the other for use among adolescents, although the MISS-R had slightly better reliability than the MISS. Additional studies are needed to determine the clinically optimal cut-score for identification of insomnia.
The longer the worse: Association between daily eating duriation and fatal cancer in Uppsala Longitudinal Study of Adult

Background
Whether the length, midpoint and day-to-day instability of the 24-h eating interval are associated with cancer morbidity and fatal cancer in older men is unknown.
Method
We used data from 941 community-dwelling men aged 71 years who participated in the Uppsala Longitudinal Study of Adult Men. Meal timing variables were derived from seven-day food diaries. We also assessed the reported daily energy intake reliability using the Goldberg method.
Results
During a mean observational period of 13.4 years, 277 men (29.4%) were diagnosed with cancer. Furthermore, 191 men (20%) died from cancer during 14.7 ± 5.2 years of follow-up. A Cox regression adjusted for potential confounders (e.g., smoking status) indicated that none of the meal timing variables significantly predicted the risk of primary cancer morbidity or fatal cancer in the entire cohort. However, among men with accurate reports of daily energy intake (n=496), each extension of the regular daily eating interval by ~80 min (one standard deviation) was associated with a greater risk of cancer with a fatal outcome (HR [95-CI]: 1.30 [1.02-1.66], P less than 0.03).
Conclusion
Our study thus emphasizes the need to consider the reliability of dietary records in nutritional cancer epidemiology. Whether curtailing the daily eating interval can be a potential behavioral strategy to reduce the risk of fatal cancer among older men warrants further investigation.
